To create a knighthood order you need to go to your castle/city and talk to your seneschal (he is stationed in your palace).
Some major cities already have knighthood orders, so you need a qualis gem and 20k. Some orders will require an NPC hero like Sir Rayne is needed to create Knights of the Falcon, if i remember correctly.
